Come, See, and Meet the Hyperbaric team at Swedish hospital.  Learn how Hyperbaric is used in other ways than just treating dive related injuries.

For those who wish to experience a ride in the chamber, you must bring a current dive *medical and your logbook with you. (Note: participants can not have any lotions, deodorants or skin care products of any kind on in order to take a chamber ride. Scrubs will be provided)

*Download medical here  You must print, complete, sign and date and bring with you if you wish to experience a chamber ride.  If you answered NO to all 10 questions on page 1 a medical evaluation is not required.  If anyone answers YES to questions 3, 5 or 10 on page one OR to any of the questions on page 2, please read and agree to the statement above by signing and dating it AND take all three pages of this form (Participant Questionnaire and the Physician’s Evaluation Form) to your physician for a medical evaluation as participation in this event requires your physician’s approval.
